Daycare, Boarding, Walking, Grooming Services Agreement

This is an Agreement between A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop, and the Owner / Guardian (as identified in the Pet Registration). Following are the terms of services for the Pet (as identified in the Pet Registration) within daycare and/or boarding, dog walking or grooming at A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop:

1. By signing this Agreement and leaving your pet with A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop, Owner/Guardian certifies to the accuracy of all verbal or written information provided about the Pet. A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop reserves the right to deny admittance to the Pet or any pet for any reason at any time.
2. Owner/Guardian specifically represents that he or she is the sole Owner/Guardian of the Pet, free and clear of all liens or encumbrances.
3. Owner/Guardian specifically represents to A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop that to their knowledge the Pet has not been exposed to any contagious diseases within the 30 day period prior to check-in. Owner/Guardian understands that each time the Pet is brought to A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop, Owner/Guardian is re-certifying that the Pet is in good health and has not had any communicable illness of any kind for 30 days prior to check-in. Owner/Guardian further agrees to provide proof of required vaccinations upon request and prior to check-in. In addition, Owner/Guardian agrees that if any fleas or ticks are discovered on the Pet during check-in, or at any time during the Pet’s stay, A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop will administer a flea bath to the Pet at Owner/Guardian’s expense.
4. A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop makes every effort to ensure that pets in daycare or boarding are healthy by requiring Owners/Guardians to affirmatively represent that all vaccines (esp. bordatello, parvo, canine distemper, parainfluenza,) for their pets are current and that they do not believe that their pets have any communicable illnesses. However, Owner/Guardian is aware and acknowledges that the employees of A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop are not veterinarians and do not have backgrounds in animal medicine, and as such, cannot be expected to diagnose or detect illnesses in the pets that are staying at A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop. In addition, Owner/Guardian is aware and acknowledges that vaccines do not protect against all communicable illnesses that may affect a pet.
5. Owner/Guardian agrees to be solely responsible for any and all acts of behavior of the Pet while in the care of A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop, including payment of costs of injury to staff or other animals or damage to facilities caused by the Pet. Owner/Guardian further agrees to indemnify A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop and its agents against any claims made against A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop or its employees or other agents, or losses or damages of any kind suffered by A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop or its agents, as a result of Owner/Guardian’s failure to inform A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop of any pre-existing condition the Pet may have (such as illness, anxiety or aggression problems) or which were otherwise caused by the Pet.
6. Owner/Guardian understands that the Pet, while in the care of A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop, will interact and co-mingle with other pets. Owner/Guardian understands that squirt water bottles, in extreme cases, pets may be muzzled for their protection or the protection of other pets and staff, or at the discretion of A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop, segregated and not permitted to interact further with other pets during the current or any subsequent stay. Owner/Guardian understands that when dogs interact and play that nicks and scratches may occur. A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop staff may or may not notify Owner/Guardian immediately if the Pet sustains any nicks or scratches, but may wait and notify Owner/Guardian when the Pet is picked up by or delivered to the Owner/Guardian.
7. A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop undertakes to perform the agreed upon services in a professional, attentive, reliable and caring manner and Owner/Guardian undertakes to provide all necessary information to assist in this performance. A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop shall supply waste bags and will promptly pick up all dog feces from all public grounds and discard in the appropriate garbage containers. Owner/Guardian will provide suitable harnesses, collars and leads that are approved by A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop as well as a jacket and muzzle if required.
8. A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop hereby warrants to keep safe and confidential all keys, remote control entry devices, access codes and any other personal information of Owner/Guardian and to return same to Owner/Guardian upon termination of this Agreement or immediately upon demand. A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop accepts no liability for any breach of security or loss of or damage to Owner/Guardian’s property if any other person is given access to the property during the term of this Agreement.
9. Owner/Guardian acknowledges that The City of Vancouver requires that all dogs be licensed according to City by-laws. Owner/Guardian understands and accepts full responsibility for non-compliance with these by-laws when leaving an UNLICENSED Pet in the care of A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop, including any and all fines, and/or apprehension of the Pet by Animal Control Officers.
10. A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop agrees to exercise reasonable care of the Pet during its stay and, if applicable, during transport. Owner/Guardian holds A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop harmless in the event of injury or accident during transportation. It is expressly agreed by Owner/Guardian and A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop that A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop’s liability shall in no event exceed the lessor of the current chattel value of a pet of the same breed or the sum of $200.00 per animal admitted.

    Owner/Guardian acknowledges and agrees that they are assuming ALL risk of illness, disease, harm or otherwise to their pet by allowing their pet to stay at A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop.

11. In an emergency, A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop will attempt to contact the Pet’s personal veterinarian and Owner/Guardian or the emergency contact provided by Owner/Guardian. However, such an emergency might not provide the time to do so prior to the administration of care.
    • Owner/Guardian authorizes A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop to obtain medical attention for the Pet from any qualified veterinarian and to transport the Pet to and from such veterinarian when A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop deems such medical care is important to the Pet’s health. Owner/Guardian grants A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop, and its employees or agents, full authority to make decisions involving the medical treatment of the Pet and agrees to pay for all related costs. This applies to any claims for injuries or damages related to such medical care or that arise due to transport of the Pet.
    • Owner/Guardian, by not providing credit card information, agrees that they are assuming all risk of harm to the Pet by authorizing A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop to make contact with Owner/Guardian prior to obtaining medical attention for the Pet.
    In the event of a Pet’s death, the Owner/Guardian or the Owner/Guardian’s emergency contact will be notified immediately.
12. Owner/Guardian agrees to pay for daycare and any additional services requested at the time Pet is picked up; Owner/Guardian agrees to pay for boarding at the time Pet is dropped off and to pay for any additional services requested when the Pet is picked up. Owner/Guardian further agrees that the Pet shall not leave the facility until all charges are paid by Owner/Guardian or payment arrangements are agreed upon by both parties.
13. The owner agrees that pre-paid daycare packages or portions there of are non-refundable and any refund is at the discretion of management of A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop. The owner agrees that A Wags Unlimited Dog Company Inc. dba The Urban Puppy Sho retains the right to expire packages after 18 months from date of purchase. The owner agrees that charges processed by Credit Card whether in person or manually are non-refundable.
14. Owner/Guardian acknowledges and agrees that this Agreement applies to the current stay and remains in effect for any subsequent stay of the Pet within the daycare and boarding services of A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop.
15. A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop adheres to a strict policy of a respectful non-bullying standards. Owners agree that we reserve the right to refuse service to any owner who abuses or is verbally abusive in any manner to any employee of A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop.
16. This Agreement contains the entire agreement between the parties. All terms and conditions of this Agreement shall be binding on the heirs, administrators, personal representatives and assignees of the Owner/Guardian and A Wags Unlimited Dog Company Inc. dba The Urban Puppy Shop.
